I acknowledge my participation in any session (private, group, camp, etc) organized by Kaizen Camps and Training LLC involves certain risks, including but not limited to bodily injury, property damage, and other dangers associated with soccer activities. In consideration of being permitted to participate in any session (private, group, camp, etc) organized by Kaizen Camps and Training LLC, I hereby agree to the following:

Assumption of Risk: I understand and acknowledge that there are inherent risks associated with soccer activities, including but not limited to collisions with other participants, contact with the ball, and playing on natural or artificial surfaces. I voluntarily assume all risks associated with my participation in any session (private, group, camp, etc) organized by Kaizen Camps and Training LLC.

Release of Liability: I release, waive, and discharge Kaizen Camps and Training LLC, its coaches, and volunteers from any claims, liabilities, damages, or demands arising out of or related to my child/children's participation in any session (private, group, camp, etc) organized by Kaizen Camps and Training LLC, including but not limited to personal injury, property damage, or wrongful death.

Indemnification: I agree to indemnify and hold harmless Kaizen Camps and Training LLC from any claims or liabilities arising out of my participation in any session (private, group, camp, etc) organized by Kaizen Camps and Training LLC including but not limited to claims brought by third parties.

Emergency Medical Treatment: In an emergency, I authorize Kaizen Camps and Training LLC and its representatives to seek medical treatment on my behalf. I agree to be financially responsible for any costs associated with such treatment.

Photography and Publicity: I consent to use photographs, videos, or other likenesses of me captured during any session (private, group, camp, etc) organized by Kaizen Camps and Training LLC for promotional purposes.

Parental Consent (for participants under 18): If the participant is under 18, I certify that I am the parent or legal guardian of the participant, and I hereby consent to their participation in any session (private, group, camp, etc) organized by Kaizen Camps and Training LLC. I agree to assume all risks and responsibilities outlined in this waiver on behalf of the participant.

I have read this waiver and fully understand its contents. I am aware that by signing this waiver, I am waiving certain legal rights, including the right to sue Kaizen Camps and Training LLC for any injuries or damages arising from my participation in any session (private, group, camp, etc) organized by Kaizen Camps and Training LLC.
